Epilepsy surgery is a valuable therapeutic option for many children with intractable epilepsy. The most effective treatment for intractable partial epilepsy is a focal cortical resection with excision of the area of ictal onset and initial seizure propagation (the epileptogenic zone). EEG monitoring continues to prove indispensable in pre-surgical planning of refractory epileptic patients by defining the epileptogenic zone. Technological progresses in both structural and functional neuroimaging techniques have led many medical centers to consider surgical treatment of epilepsy. In children undergoing pre-surgical evaluation, the goals of neuroimaging studies include the identification of structural abnormalities in the brain, eloquent regions of the brain including language, memory, and sensorimotor functions, and the relation of these regions to the potential epileptogenic region. Neuropsychological testing plays a crucial role in assessing the potential impact of surgery on cognitive function of the patients and helps in lateralizing the cerebral hemisphere dominant for verbal and nonverbal function in older children. The Wada test is an invasive procedure to determine language dominance and can be used to assess the risk of postoperative memory deficits in children with temporal lobe epilepsy. Some children scheduled for resection still need to undergo further precise localization of the epileptogenic zone and functional mapping with invasive EEG monitoring through intracranial subdural grid and/or depth electrodes. Epilepsy surgery has the potential of changing the life quality of children by improving or eliminating seizures in carefully selected patients.

CAUSES OF INTRACTABLE SEIZURES IN A SURVEY OF 10,000 EPILEPTIC CHILDREN
Although treated properly for epilepsy, a number of epileptic children have intractable seizures and about 5-10 per cent of them do not respond to anti -epileptic drugs. In the years 1986-1996 about 10,000 epileptic patients were treated in the Children's Medical Center in Tehran. A retrospective study of their medical records revealed that 41 of them had intractable seizures. I Of these 4...

Introduction: about 20% to 30% of patients with epilepsy are diagnosed with drug-resistant epilepsy and one third of these are candidates for epilepsy surgery. Surgical resection of the epileptogenic tissue is a well-established method for treating patients with intractable focal epilepsy. Determining language laterality and locality is an important part of a comprehensive epilepsy program befo...
متن کاملHIGH-DOSE ORAL PYRIDOXINE FOR TREATMENT OF PEDIATRIC RECURRENT INTRACTABLE SEIZURES
Intractable epilepsy is a common clinical problem in pediatrics and approximately 13% of children with epilepsy experience intractable seizures. To determine the efficacy of pyridoxine in treating seizures, 30 infants and children with recurrent seizures were enrolled in the present study. All of them were treated with high-dose oral pyridoxine (40 mg/kg/day), as an adjunct to antiepileptic...

Abstract Introduction: The present study was conducted with an eye on investigating the effects of Acceptance and Commitment Therapy (ACT) on seizure frequency and self-management behaviors of patients with intractable epilepsy. Methodology: Quasi experimental design was adopted in which pre-test, post-test, and control group were deployed to gather data from patients with epilepsy visiti...
